LIBRARY CARD APPLICATION
Residents of the Village of LaGrange are entitled to a library card upon showing proof of residence. Signature of a parent or guardian is required to obtain a card for children under 18 years. The La Grange Public Library card is also honored in any library of the Metropolitan Library System and most Illinois public libraries.
To OBTAIN a new library card:
Bring two formsof identification with your current address to the Patron Services desk. Some examples:
· property tax bill
· lease
· current electric or gas bill
· driver's license or state ID
· bank statement or checkbook with imprinted checks
To RENEW a library card:
Bring expired card and one piece of identification with your current address to the Patron Services desk. If you have lost your card, two ID's will be required, and a $2 replacement fee may be charged.
